How We Collect Personal Information
We collect your personal information directly from you whenever reasonably possible, including when you make an inquiry or order through our website (leafdoctors.com.au), during consultations (in person, by phone, or via telehealth), through email or written correspondence, by completing online forms or questionnaires, and through patient support services, surveys, or feedback.
We may also collect your personal information from third parties, such as other treating healthcare providers (GPs, specialists, allied health), pharmacies and partner practitioners, and family members, carers, or guardians (with your consent).
Why We Collect Your Personal Information
We collect and use your personal information for the purposes of providing you with medical care and treatment, coordinating care with other healthcare professionals involved in your treatment, prescription dispensing and supply (including sharing your initials and date of birth with partner pharmacies and suppliers), administrative functions including appointment scheduling, patient record management and billing (including Medicare compliance), sending reminder notices via SMS and/or email regarding treatment follow-ups or recalls, monitoring and tracking your treatment progress, conducting medical tests, referrals, or external specialist care as required, compliance with legal obligations including court orders, subpoenas, or notifiable disease reporting, disclosure to law enforcement agencies for investigations, providing de-identified information for research purposes, and communicating with authorised guardians or persons nominated by you regarding your treatment.
Overseas Disclosure
To provide timely and efficient care, Leaf Doctors may engage administrative and clinical support staff located outside of Australia. Currently, some contracted staff are located in the Philippines and may access personal information for purposes such as appointment scheduling, record management, and patient support. We take steps to ensure that all offshore staff are bound by confidentiality agreements, adhere to privacy and security standards equivalent to those required under Australian law, and receive ongoing privacy and security training. By continuing to use our services, you acknowledge that your personal information may be accessed by authorised personnel outside Australia.
SMS and Email Communications
Leaf Doctors may contact you by SMS or email for appointment reminders, treatment-related communications, information about medical services provided by Leaf Doctors, and marketing or promotional messages (from Leaf Doctors only). If you do not wish to receive certain types of communication, you may opt out at any time by contacting us in writing.
Security of Your Personal Information
We apply administrative, technical, and physical safeguards designed to ensure confidentiality, integrity, and availability of personal health information, protect against unauthorised access, misuse, or disclosure, and maintain secure storage and encrypted communication channels. Access to your personal information is restricted to authorised staff and practitioners who require it for the provision of care or support. We retain your personal information for as long as legally required (generally a minimum of 7 years) and securely destroy or de-identify it when no longer needed. Please note: Leaf Doctors records telephone calls for training purposes. If you do not wish for your call to be recorded, please advise our staff at the start of the call.
Access, Correction, and Complaints
You have the right to request access to your personal information, request corrections or updates to your personal information, and complain if you believe your privacy has been breached. Requests should be made in writing to our Privacy Officer:
